Documents You’ll Need to Replace Your Driver’s License

Two documents from the list of primary documents to prove your identity – these documents will need to show your legal name and your age. These documents can be your U.S. or Canadian birth certificate, a valid, unexpired U.S. passport, and a driver’s license with a color photo that has been expired less than five years.

OR one document from the primary list and one document from the secondary list of documents, which include a U.S. Social Security card or a Canadian Social Insurance card, a certified marriage certificate, or a Medicare, Medicaid, or health insurance card that includes your name and insurance ID number.
